Who may be referred?
Any child aged birth to school leaving age who attends nursery or school in Camden.
Who can refer?
- Teachers, SENCOs, school nurses
- School/hospital doctors, GPs
- Allied health professionals
- Parents
How to make a referral
If you are a parent of a school aged child in Camden speak to your child's teacher, or the SENCO at your child's school, who will normally be happy to discuss a referral to our service with you. If your child is not at school please call for further advice.

Please download and complete the teacher questionnaire after you have obtained consent for the referral from the child's parents.
The occupational therapy department will normally send out the parent quetionnaire (and the
letter of consent) once we receive the referral form.
Once a referral is accepted the child will be placed on our waiting list and the referrer will receive an acknowledgement letter.
